Kratom is sold legally in California, in smoke shops, gas stations and online, as a powder, a capsule, an extract or a drink. That legality is why so many people are surprised by what happens when they try to stop. Kratom contains active alkaloids, particularly mitragynine and 7-hydroxymitragynine, that interact with opioid receptors. Regular use can produce physical dependence and an opioid-like withdrawal syndrome, although the clinical picture varies considerably with the product, concentration, amount and frequency of use. The withdrawal is real, it is an opioid-type withdrawal, and it deserves to be treated as one.

Why Kratom Withdrawal Is Underestimated
Kratom comes from the leaves of a Southeast Asian tree. Its main active compounds, mitragynine and 7-hydroxymitragynine, bind to the same opioid receptors as morphine and oxycodone. At low doses kratom feels stimulating. At higher doses it is sedating and pain-relieving, and that is the range most people who become dependent are using in.
Many people started using kratom to get off something else: opioid painkillers, heroin or alcohol. Others started for pain, energy, anxiety or focus after reading that it was natural and non-addictive. Over months the dose climbs, the gaps between doses shrink, and stopping produces the same restlessness, aches, sweating and cravings that opioid withdrawal does. Because it is legal and plant-based, people often push through alone for days, assume the symptoms mean something else is wrong, or go back to using because nothing else stops it.
Concentrated extracts and the newer 7-hydroxymitragynine products sold as “7-OH” tablets are far stronger than leaf powder. Dependence on them develops faster and withdraws harder. We are seeing more of these, and they are treated with the seriousness of any opioid.
Kratom Withdrawal Symptoms
Withdrawal symptoms may begin within a day after stopping, but onset, severity and duration vary with the product, dose, frequency and duration of use. Concentrated extracts and 7-OH products may produce a different and potentially more severe withdrawal course than traditional kratom leaf.
- Restlessness and an inability to sit still
- Muscle aches, especially in the legs
- Runny nose, watery eyes, yawning
- Sweating, chills and goosebumps
- Nausea, stomach cramps and diarrhea

- Anxiety, irritability and low mood
- Intense cravings
Kratom withdrawal is not usually life-threatening on its own, but dehydration, other medical conditions and withdrawing from more than one substance all change that picture. Assessment is what tells us which situation you are in.
7-OH and High-Potency Kratom Products
Not all products sold under the kratom umbrella are comparable. Traditional kratom leaf naturally contains only small amounts of 7-hydroxymitragynine (7-OH). Newer concentrated 7-OH products can deliver substantially higher amounts and may produce significant opioid-like effects, physical dependence and withdrawal.
At Revive, we specifically ask whether someone is using traditional leaf or powder, extracts, shots, tablets, gummies, or concentrated 7-OH products. Someone dependent on a high-potency 7-OH product should not assume their withdrawal will resemble withdrawal from traditional kratom powder. The exact product, amount, frequency and duration of use, along with other substances and medical conditions, all inform the withdrawal-management plan.
How Kratom Detox Works at Revive
- Assessment on arrival. What form of kratom you use (powder, capsules, extract, 7-OH products), how much and how often, when the last dose was, and whether anything else is in the picture. Kratom is often used alongside alcohol, benzodiazepines or opioids, and that changes the plan. Your medical history, current medications and how you feel right now. Vital signs and a withdrawal scale give the medical team a baseline.
- Medication to make withdrawal bearable. Because kratom and concentrated 7-OH can produce opioid-like dependence and withdrawal, treatment may incorporate approaches used in opioid withdrawal management, individualized to the patient’s presentation. Depending on the assessment, that may include a medication-assisted treatment approach that eases withdrawal directly, along with comfort medications for sleep, nausea, cramping and anxiety. What is prescribed, and for how long, is a medical decision made with you.
- Monitoring around the clock. Symptoms, vital signs and fluid intake are checked regularly, day and night. With six beds, changes in symptoms can be recognized and addressed promptly by the care team.
- A house, not a ward. Meals prepared for a body that has not been eating, a quiet bedroom, a garden, and when you feel able, the living room where our meditation, music, PEMF and red light sessions happen. Nothing is required of you during the worst of it.
- The handoff. Before you leave we agree on what comes next: residential treatment here in the same house, continuing medication-assisted treatment, or a program closer to home. If kratom was standing in for something else, the plan has to account for that, or the next relapse is to the original drug.

If Kratom Was Your Way Off Opioids
Many of the people we see used kratom to get off oxycodone, heroin or fentanyl, often on their own, and often successfully for a while. Then the kratom became the dependence. There is nothing to be ashamed of in that. It was an attempt at recovery with the only tool available. The difference now is that you can do it under medical supervision, with medication designed for opioid-type withdrawal, and with a plan for the pain, anxiety or trauma the original drug was covering. That is a better second attempt, not a return to square one.
One safety note that applies to anyone who has used opioids: after detox, tolerance drops fast. If someone returns to fentanyl, heroin or another opioid after a period of abstinence, reduced opioid tolerance can substantially increase overdose risk. We say this to every person and every family before discharge. Naloxone (Narcan) reverses opioid overdose, is available without a prescription in California, and anyone living with a person in recovery should have it.
Who Kratom Detox Is For
Anyone using kratom every day who has tried to stop and could not get past the second day. Anyone whose dose has climbed to the point of using every few hours, or who has moved from leaf powder to extracts or 7-OH. Anyone using kratom to manage withdrawal from another opioid. And anyone who was told, by a shop or a forum, that kratom is not addictive and now knows otherwise.
